• Played at a high school or college level. (This can be waived
  in some cases)
  
  
• Willing to obtain a USSF F and E license. (Paid for by our club.)
  
  
• Willing to work toward a USSF state D license.
  
  
• Willing to commit to a program for individual player development.
  A program where players are brought to the next level.
  
  
• Willing to attend regular coaches meetings and coaching
  seminars held by our club.
  
  
• Willing to be a positive role model.
  
  
  

Below are the 2 pages of the player evaluation form.  Please evaluate the player on their progress.  Include in your comments any positive feedback you can provide and what they need to do to improve.

If possible, please also try and have a one on one discussion with the player regarding their evaluation.
